Terms and conditions for FIT4MOM Unlimited

This contract is made between the client and FIT4MOM DuPage County and is a month to month agreement after the initial 3 month commitment and except as provided in the initial 10 day cancellation policy below, may be terminated by either party with the proper advance notice. This membership allows you to attend unlimited Stroller Strides, Stroller Barre, Strides 360 (360), Stroller Mash-up, BOOST, and FIT4MOM Strength classes (Body Ignite).

AUTHORIZATION FOR PAYMENT

I authorize HZ HOLDINGS LLC (FIT4MOM DuPage County) to initiate a charge to the account entered into Pike13 for my dues. This authorization is to remain effective until I have cancelled my membership through the FIT4MOM DuPage County administrator.

I have the right to stop payment on an Electronic Funds Transfer debit by notifying my bank. This, however, does not void my contract with FIT4MOM DuPage County to fulfill my payment commitment and I am obligated to pay. Dues will be drawn monthly from the account on file on the 1st of each month. The processing date for debit cards may vary due to banking procedures. If charges are returned they will be subject to a late fee of $10. Members are responsible for updating their credit card information online or with a staff member. Members may not attend class who have unpaid balances. If a membership remains unpaid it will be cancelled.

CANCELLATION POLICY

All membership cancellations must be received (via this form) to cancel for the following month. A minimum of 10 days notice from payment due date after the initial 3 month commitment is required.

You will receive an email confirmation of the cancellation of your membership.

All cancellations will occur on the due date of your billing cycle.

Dues paid are not refunded. All requests are for future months only.

Our instructors cannot authorize hold or cancel requests, either verbally or in writing. Holds or cancellations will only take place for upcoming months. We will not go back and credit dues already paid. It is your responsibility to make sure we have received the written confirmation that your status change request was received and will be put into place.

Please allow our staff up to 48 hours to contact you to confirm your cancellation request.

Once a membership is cancelled, if the member wants to re-enroll they will be charged a $25 reactivation fee.

All cancellations prior to the initial 3 month minimum commitment will be subject to a $49.50 early termination fee.

HOLD POLICY

Memberships may be held a maximum of 4 months per hold period. To initiate a hold please complete this form.

Membership holds require a 15 day notice from your payment due date. If less than 15 day notice is given you will be charged a $20 administrative late notice fee. Hold starts will coincide with the next date dues is scheduled to be drawn out on and can end on any day of the month.

All hold periods will be set up for 4 months unless otherwise requested.

Dues payments automatically resume at the end of the hold period. We do not send notification prior to the payments resuming. It is imperative that you stay on top of your Membership status with csdupagecofit4mom@gmail.com if you have any questions about the duration of your hold.

To return prior to your hold period ending, email csdupagecofit4mom@gmail.com with your exact date of return to class or just show up at class and inform a staff member. This will be the first day that your dues payments resume. It can be on any day of the month and dues will be pro-rated.

To extend your hold period, you can email

by 2 weeks prior to your dues payments being scheduled to resume so that we can adjust your hold period before your dues payment happens.

To attend any classes while your membership is on hold, you can pay the drop-in rate of $30/class. Just let your instructor know at sign in.

Please allow a staff member up to 48 hours to contact you to confirm your hold.

The only exception to this hold policy are expectant moms who deliver and plan to return to class after recovery can be put on hold immediately (after notice is given via writing).

INITIAL 10 DAY CANCELLATION POLICY

You, the buyer, may cancel this agreement at any time prior to midnight of the tenth (10th) calendar day after you have signed this agreement. To cancel this agreement written notice must be received by FIT4MOM DuPage County within the initial ten days. You are responsible for paying for the classes already attended and equipment given to you.

UPGRADED MEMBERSHIPS

Should a client choose to upgrade their membership from another FIT4MOM DuPage County plan they will be required to meet the 3 month minimum commitment. If you need to cancel or change your policy again, the same 30 day notice is required.

AGREEMENT

By purchasing this contract, you are agreeing to the terms of this contract. Terms of this agreement may change with proper advance written notice.
